Disabled Citizen Reduced Fare ID Card


Persons with disabilities must complete an “ID Card Request Form.” This form is available at the ECAT Administrative Office, Customer Service, or by downloading the PDF file using this link. Mail, drop off, or fax the form to 850-595-3222. After the completed form is received, ECAT will obtain required information from your physician. Upon receipt of the physician’s verification of disability, ECAT will notify you and you may purchase an ECAT Reduced Fare ID card. To check on verification, call 850-595-3228. ECAT Reduced Fare ID cards may be purchased immediately if the individual has a Medicare card, a Social Security statement verifying disabled income, a Veteran’s Affairs (VA) card or if the individual’s social worker, counselor, or support coordinator is present. The social worker, counselor, or support coordinator must present identification. You must present your ECAT Reduce Fare ID card to pay reduced fare or when purchasing and using the reduced passes or tickets.
